From Hospital to Home

Why discharge planning matters

A poorly planned discharge can mean readmission, lost skills or an unsafe living situation. We work with discharge planners, participants and families to have supports ready on day one — not day ten.

How it works

  • Referral — hospitals, support coordinators and families can refer at short notice; we run same-week capacity checks
  • Planning — we confirm the destination (home with supports, STA, MTA or a longer-term home), check funding covers day-one needs, and flag urgent plan reviews early
  • Clinical handover — our nurses take handover from the ward for medication, wounds and catheter care
  • Familiar faces — wherever possible you meet your support team before discharge
  • Day 14 review — we front-load support in the first two weeks, then step it up or down based on what’s really happening at home

If home isn’t ready yet

We can support short-term accommodation (STA) and medium-term accommodation (MTA) stays while a long-term home is arranged — see our respite and short stays and current available homes.
