We support people living with disability to live with independence, dignity and choice. We deliver compassionate, culturally responsive support — from everyday help at home to complex and high-intensity care — and build genuine, lasting relationships with the people we support, their families and their support teams. We focus on getting the details right: consistent support workers who actually know you, clear and timely communication, and careful planning that respects each person's goals and right to make their own decisions.

What we do

Support built around each person

We provide individualised support and care so people can live well in their own homes and communities. Our core services include:

  • Supported Independent Living (SIL): help with daily routines, personal care and skill-building at home, including overnight and 24/7 support.
  • Specialist Disability Accommodation (SDA): supporting eligible participants in purpose-built, accessible homes.
  • Complex & high-intensity care: clinically overseen support for participants with complex health and behaviour support needs.
  • Community access & participation: support to get out, stay connected and take part in community life.
  • From Hospital to Home: coordinated discharge support that can start at short notice.
  • Forensic supports: specialised, risk-informed support for participants with justice-system involvement.

Vitality Community Care was built around a simple idea: support works best when it comes from people who know you — familiar faces who understand your routines, your goals and your community.

From our first participants in Melbourne, we’ve grown into local teams across Victoria, New South Wales and Western Australia. What hasn’t changed is how we work: consistent support workers, clear communication, and planning that respects each person’s right to make their own decisions.
