Community Nursing

Clinical care at home from registered nurses — medication, wound care, continence and ongoing health needs, coordinated with your GP and specialists.

A registered nurse providing care at home

What our nurses do

  • Medication management and administration
  • Wound care and pressure injury prevention
  • Continence assessment and catheter care
  • PEG feeding oversight and enteral nutrition
  • Diabetes support and health monitoring
  • Post-hospital clinical handover and follow-up

Nurse-led, not nurse-only

Our registered nurses don’t just deliver care — they train, delegate to and supervise the support workers around you, sign off competencies for high-intensity supports and keep your health support plans current with your treating team. That clinical oversight is what makes complex care at home safe.

Where it happens

In your own home, in our supported accommodation, and as part of hospital-to-home transitions across Melbourne, Sydney and Perth.

Good to know: community nursing is typically funded through your Core or Capacity Building budgets depending on the support — we’ll help you check.
